NOTICE!

 

LEGISLATION THAT BECAME EFFECTIVE JULY 1, 2003 REQUIRES STRICT SECURITY MEASURES WHEN REQUESTING CERTIFIED COPIES OF BIRTH OR DEATH RECORDS.

MARRIAGE LICENSES ARE SUBJECT TO THE SAME REGULATIONS AS BIRTH AND DEATH CERTIFICATES - EFFECTIVE JANUARY 1, 2010. 

THE ONLY PERSONS AUTHORIZED BY LAW TO RECEIVE A CERTIFIED COPY OF A BIRTH, DEATH, OR MARRIAGE RECORD ARE:

  • THE REGISTRANT OR A PARENT OR LEGAL GUARDIAN OF THE REGISTRANT
  • A PARTY ENTITLED TO RECEIVE THE RECORD AS A RESULT OF A COURT ORDER, OR AN ATTORNEY OR A LICENSED ADOPTION AGENCY SEEKING THE BIRTH RECORD IN ORDER TO COMPLY WITH THE REQUIREMENTS OF SECTION 3140 OR 7603 OF THE FAMILY CODE.
  • A MEMBER OF A LAW ENFORCEMENT AGENCY OR A REPRESENTATIVE OF ANOTHER GOVERNMENTAL AGENCY, AS PROVIDED BY LAW, WHO IS CONDUCTING OFFICIAL BUSINESS.
  • A CHILD, GRANDPARENT, GRANDCHILD, SIBLING, SPOUSE, OR DOMESTIC PARTNER OF THE REGISTRANT.
  • AN ATTORNEY REPRESENTING THE REGISTRANT OR THE REGISTRANT'S ESTATE, OR ANY PERSON OR AGENCY EMPOWERED BY STATUE OR APPOINTED BY A COURT TO ACT ON BEHALF OF THE REGISTRANT OR THE REGISTRANT'S ESTATE.
  • ANY FUNERAL DIRECTOR ORDERING CERTIFIED COPIES OF A DEATH CERTIFICATE ON BEHALF OF AN AUTHORIZED INDIVIDUAL LISTED ABOVE.

EVERY PERSON WHO REQUESTS CERTIFIED COPIES OF BIRTH, DEATH, OR MARRIAGE CERTIFICATES IS REQUIRED TO PRESENT PHOTO IDENTIFICATION AND SIGN A SWORN STATEMENT UNDER PENALTY OF PERJURY THAT THEY ARE AUTHORIZED TO RECEIVE A CERTIFIED COPY. REQUESTS RECEIVED BY MAIL FROM NON-GOVERNMENTAL AGENCIES MUST INCLUDE THE NOTARIZED STATEMENT SWORN UNDER PENALTY OF PERJURY.

To obtain copies of your vital records, you may use our online VitalDirector service, apply in person in our office, or apply by mail using the forms below.

** DISCLAIMER:  If the requested record is not found, you will be issued a Certificate of Failure to Find, and all application fees will be retained under Health & Safety Code Section 103650.
