Provider orientation will give you important information about the IHSS Program and the rules and requirements you must follow as a provider. To complete orientation Online, follow the steps below:

                 SEIU 2015 - Together we rise English 
                 SEIU 2015 - Together we rise Spanish 

3. Upon completion, you must sign and mail the IHSS Program Provider Enrollment Agreement 

                 Agreement: SOC 846 English
                 Agreement: SOC 846 Spanish
