NOTICE!

LEGISLATION, EFFECTIVE JANUARY 21, 2004 REQUIRES STRICT SECURITY MEASURES WHEN REQUESTING CERTIFIED COPIES OF MILITARY DISCHARGE DOCUMENTS (DD214).

AFTER JANUARY 21, 2004 THE ONLY PERSONS AUTHORIZED BY LAW TO RECEIVE A CERTIFIED COPY OF A MILITARY DISCHARGE DOCUMENT (DD214) ARE:

  • THE PERSON WHO IS THE SUBJECT OF THE RECORD UPON PRESENTATION OF PROPER PHOTO IDENTIFICATION.
  • A FAMILY MEMBER OR LEGAL REPRESENTATIVE OF THE PERSON WHO IS THE SUBJECT OF THE RECORD UPON PRESENTATION OF PROPER PHOTO IDENTIFICATION.
  • A COUNTY OFFICE THAT PROVIDES VETERAN'S BENEFITS SERVICES.
  • A UNITED STATES OFFICIAL UPON WRITTEN REQUEST.

REQUESTS RECEIVED BY MAIL FROM NON-GOVERNMENTAL AGENCIES MUST INCLUDE THE NOTARIZED STATEMENT SWORN UNDER PENALTY OF PERJURY.
