Military Discharge Document Request
NOTICE!
LEGISLATION, EFFECTIVE JANUARY 21, 2004 REQUIRES STRICT SECURITY MEASURES WHEN REQUESTING CERTIFIED COPIES OF MILITARY DISCHARGE DOCUMENTS (DD214).
AFTER JANUARY 21, 2004 THE ONLY PERSONS AUTHORIZED BY LAW TO RECEIVE A CERTIFIED COPY OF A MILITARY DISCHARGE DOCUMENT (DD214) ARE:
- THE PERSON WHO IS THE SUBJECT OF THE RECORD UPON PRESENTATION OF PROPER PHOTO IDENTIFICATION.
- A FAMILY MEMBER OR LEGAL REPRESENTATIVE OF THE PERSON WHO IS THE SUBJECT OF THE RECORD UPON PRESENTATION OF PROPER PHOTO IDENTIFICATION.
- A COUNTY OFFICE THAT PROVIDES VETERAN'S BENEFITS SERVICES.
- A UNITED STATES OFFICIAL UPON WRITTEN REQUEST.
REQUESTS RECEIVED BY MAIL FROM NON-GOVERNMENTAL AGENCIES MUST INCLUDE THE NOTARIZED STATEMENT SWORN UNDER PENALTY OF PERJURY.
