The front side of our Vote-by-Mail Return Envelope (VBM) will look like this:

1. This is an accessibility tool to help voters with low vision locate the signature line.

2. This is where you sign your envelope. We will match this signature to what is in your voter profile. The return envelope must be signed by the Voter in order for us to process the ballot.   

3. This is where you should write your physical address.  Please do not write your mailing / P.O. Box address.

4. This is where you should write your City, State, and Zip code. 

5. If a voter cannot sign their Ballot Return Envelope, they would make their "mark" on line 2.  The person who witnessed the mark being made must sign here. 

6. If a voter designates a person to return their ballot on their behalf, that name should be written here.

7. The person who has been designated to return the ballot should sign their name here.

 8. These are general reminders to the voter regarding what to do if they need a replacement ballot and to make sure that both the ballot is placed into the envelope and the envelope is signed before sealing. This is located underneath the sealing flap.

9. This is the back side of the accessibility tool used for signing the Ballot Return Envelope.  The Elections Office also uses this to make sure that all contents of the envelope have been removed during ballot processing.  

10. This is the ballot issue ID number.  This number is used to track the ballot mailed to you through out ballot processing and to give you voting credit when the ballot envelope is returned and accepted. Please note that once the ballot has been separated from the envelope, we are unable to track an individual's ballot.  This is to maintain secrecy of the vote.

11. This is your master voter ID number.  This is how we track all voters in Kings County.  No two voters have the same Voter ID. 

12. This is the address the ballot was mailed to.  This can be different than your residence address.
